Create and post a form page for your final site. Link it from the words "Form exercise" on your Course Work page.
(if you do not actually need a form for your final site make up a form for a fictitious company).

Your form should include:

• the form tags including URL of your script
• a text field
• a text area
• a two radio buttons
• a check box
• a pull down menu
• the reset and submit buttons

In addition to the form elements, this page should include all the elements, essential to each page in a site, that we have covered so far in the course:

• header
• footer
  - this includes text-based navigation menu with home link and link to other sections of site and copyright info.
